The CARE Point team are Social Workers and Community Wellbeing Officers based in North Tyneside General Hospital, working as part of a multi-disciplinary service alongside discharge nurses and occupational therapists.

The team work closely with our Reablement team, completing assessments and support planning for people who may have ongoing social care needs following reablement intervention.

CARE Point aims to prevent hospital admissions, and facilitate safe discharge from hospital. We aim to support people to return home wherever possible, or to appropriate step-down care. 

  • CARE Point facilitate all hospital discharges for North Tyneside residents who do not have an existing service arranged by Adult Social Care;
  • Referrals for assessment are made directly from hospital wards.

Within CARE Point, there are smaller cluster teams, all based within North Tyneside General Hospital:

  • Two clusters work into wards in North Tyneside General Hospital;
  • One cluster supports North Tyneside residents who are admitted to hospitals outside the borough, plus supporting people to prepare for their next steps after intermediate/’step-down’ care.
